Counseling Referral Services

Purpose

The purpose of this Memorandum of Understanding ("MOU") is to establish mutual understanding about the counseling relationship and address attendance, financial responsibility for missed appointments, and timekeeping requirements related to counseling sessions.

For the Employee

We’re glad you’ve made the decision to attend counseling. It’s a wonderful resource and has benefited many employees and their families, and we pray that it is a helpful resource for you as well.

If you are an hourly employee, clock out when you leave and clock in when you return. In the ADP app, log the time you missed at AGI as Ministry Pay. You will also need to let your supervisor know that you have a personal appointment. If they have any questions they can speak with the Ministry Department.

If you are a salaried employee, you do not need to clock out. However, you do need to let your supervisor know that you have a personal appointment. If they have any questions they can speak with the Ministry Department.

Attendance and Missed Appointments

Employees or their family members are expected to attend all scheduled counseling appointments. If you need to cancel a counseling appointment, please do so at least 24 hours in advance. Before canceling, ask if they are available for a virtual visit or can reschedule, this will usually avoid any cancellation fees.

If an employee or their family member misses more than two (2) counseling appointments within a three (3) month period, the employee will be financially responsible for any fees associated with future missed appointments, as charged by the counseling provider. If the employee or their family member has missed two (2) consecutive sessions, there is a likelihood that you will lose your appointment time and need to reschedule. 

Missed appointments include no-shows or late cancellations as defined by the counseling agency’s policies.

Confidentiality

All counseling services are confidential between the employee or their family members and the counseling provider, subject to applicable laws. AGI Ministry will not receive details of counseling sessions, except as required for scheduling, billing, or compliance purposes. AGI Ministry will be notified if the counseling agency believes that the employee or family member poses a threat of harm to themselves or others.

Acknowledgment and Agreement

By typing my name below, the employee and/or their family members acknowledge understanding of and agreement with the terms of this MOU.

By typing my name at the bottom of this document, I give AGI Ministry Staff permission to communicate directly with my counseling agency or counselor concerning billing and receipts and to make payments on my behalf and give Global Counseling Network permission to share information about the frequency of my counseling appointments and billing information related thereto.
